-Details of the applicant (company, First Name, Last Name, Address, Phone Number)
Description of the anomaly
Attach the following photos:
-Photo Product seen in its totality
-Photo Of the particular defective
-Photo Holographic label is readable from which the quality control code "QC"
Text and photos must be contained in a single email message.
